Freedom of Information Act 2014

The Freedom of Information Act 2014 is designed to allow individuals access to information held by public bodies which is not routinely available from other sources.

St Vincent's University Hospital fosters a culture of openness and transparency. The Freedom of Information Act asserts the right of members of the public to obtain access to official information to the greatest extent possible consistent with the public interest and right to privacy of individuals.

Information in this regard may include:

1. Requests for Patient Information
2. Requests for Information of the Deceased
3. Requests for Non-Personal Information

1. Request for patient information

Freedom of Information is not required to request access to information contained a patient’s own medical records. These requests can be made by written application including the following details:

  • Family name
  • First name
  • Current Address
  • Previous Address (if applicable)
  • Phone number
  • Sufficient details to allow the hospital to locate the medical records
  • Details of the records requested
  • The reason for the request. This is to enable staff to assign the request to the right department.
  • Two forms of identification, one to include photographic identification.

2. Request for information of the deceased

The next of kin, or other family members, may request access to a deceased patient’s medical records. These requests are always dealt with by the Freedom of Information Department who will balance the patient's right to confidentiality with the right of another person to be given the information in the interest of the public good.

In order to make a request, the following will initially be required for consideration:

  • State that your request was made under the Freedom of Information Act.
  • Identify the records or information you require.
  • Provide your full personal contact details and also the details of the deceased in order to locate the information being requested.
  • Provide a copy of two forms of identification, e.g. Passport or Drivers Licence and Utility Bill or Bank Statement.
  • Proof of the relationship to the deceased person and a copy of the deceased person’s death certificate.
